The following is a sample of our computer simulations involving industrial accidents:

Tortilla Press

The guarding and operation of a tortilla press is discussed.  Worker's injuries are not consistent with the manner described by the worker. 

Paper Rewinder

An employee at a paper plant is instructed to wrap two rolls of paper using a single sheet of plastic, then slit the plastic with a utility knife to create two separate rolls.  The nip point is within the employee's work area and his fingers are caught.  A safer alternative design is shown.

House Fire

An improperly installed venting system for a fireplace results in fire inside a home. This simulation goes over the details of the fireplace system and compares a proper installation versus the actual installation and how that installation caused the fire.

Tire Cage Accident

Two versions of a person inflating a tire using a tire cage. In one case, the tire deflates but the cage protects the person without incident. In another case, the tire deflates and the cage bounces off of a wall, resulting in injury.

Grinder Accident

Machinery misuse. An employee at a cheese factory tried to collect samples from the output of a cheese grinder after the guard had been removed from the machine. Due to the combination of big cheese clumps falling onto his hands, cold hands, and a CO2 cloud, the employee lost sight of his hand position under the grinder.

Elevator Accident

A woman exiting an elevator is hit by the closing doors. She is either hit by the doors as they close, or she walks directly into the doors as they close.

Backhoe Accident

Industrial. A backhoe operator was digging a hole next to a telephone pole in order to remove it. While digging, the soil around the pole loosened and fell away causing the pole to fall onto the cab of the backhoe.

Tortilla Extruder

Improper Guarding. Improper design allows employee to improperly place arm into machine.

Gate Accident

Gate. The front section of a manual “piggyback” sliding gate fell on the plaintiff and caused a fatal injury. The clip shows how the gate, being misaligned off its tracks, can miss the coupling stopper and become free.

Drill Platform Fall

While trying to give orders on a job site, a foreman loses his balance and falls. Different versions depict several possible fall directions.
